Basic Background Information

Who

·      Who is involved with this problem?

·      Who is responsible for resolving it?

What

·      What about this situation is most urgent and needs attention.

·      What are possible root causes for this situation?

Why

·      Why does this situation need to be resolved?  IF you don’t solve it, what might happen?

When

·      When did the problem first appear?

·      When will the problem stop being a problem?

Where

·      Where is this situation most noticeable?

·      Where is the situation least noticeable?

How

·      How will you know when the problem is solved?

·      How might the root cause be resolved?

 

Transient (Change) Data

Who

·      Who was involved at the time this situation started? 

·      Who left or was brought on just before things changed?

What

·      What events led up to the present problem? 

·      What change was made just before the problem started?

·      What successful actions stopped or dropped off? 

Where

·      Where did the problem come from?

·      Where have new or revised policies, practices or procedures been put in place?

·      Where have priorities shifted? 

When

·      When did the problem start? 

·      When was the problem first noticed?

Why

·      Why does this problem continue to exist?

·      Why hasn’t it been resolved?

How

·      How has the workload shifted?

·      How pervasive or widespread is the problem?

·      How quickly is the problem spreading or developing?
